Patient Info.

What to bring to your appointment.

  • Your current insurance card (need subscriber's name, DOB, Soc. Sec. #)
  • Guarantor or employer's information (name, address, & phone number)
  • Physician order forms (Physician ordering test must be Texas State Board Certified)
  • Social Security number or card
  • Picture ID with current address (Driver's License or Texas ID)
  • A list of current prescription or over-the-counter medications you are taking, including the dose and frequency
  • Copy of Advance Directive (only if patient is being admitted, having a surgery/outpatient procedure or is an OB outpatient)

**Pre-registration is recommended for patients with scheduled procedures (ex: CT, MRI, Mammogram, Sonogram, Day Surgery) or outpatient procedures (ex: EGD, Blood Transfusions, Chemo, Cath and labs).

We make every effort to remain on schedule, however because of hospital emergencies, we occasionally get behind.  If this occurs, we ask for your cooperation and understanding.

HIPAA

HIPAA (Health Insurance Portability and Accountability Act) is a federal law created in 1996 to protect the patient's rights in regards to their health information.

Insurances Accepted

Our Hospital is a provider for Medicare, Medicaid and most major insurance plans.  Be prepared to pay your co-payment and/or deductible at the time of registering.  We also offer hospital based financial assistance programs for health care services through our Social Services Department.

Advantage Care

UMH offers a wellness benefits program to you and your family that focuses on providing health benefits, preventative screenings and savings on your out-of-pocket expenses for wellness exams.
