Medication Assistance Program

The Social Services department helps patients who apply for the medication assistance program.  Medication assistance programs are designed for uninsured patients. Social Services accesses assistance through the pharmaceutical companies who donate their medications.  There is an application process in which the patient provides their proof of income, and together, a Social Services representative and the patient fill out the application and forward it to their prospective doctor who signs it, attaches the appropriate prescription and mails in the attached envelope.  If the patient meets the requirements, they receive a 90 day supply and are eligible for assistance for an entire year.  Patients are asked to contact Anita Bass for their next refill and then a new application when necessary.
