Community Health Needs Assessment
A Community Health Needs Assessment captures the community's perspective of needs through the analysis of information obtained over the course of several months and multiple research methods.
Through the Internal Revenue Service, the Federal Government requires non-profit hospitals to conduct a CHNA every three years. These community-based studies help health care providers identify local needs, determine the availability of local resources, prioritize strengths, weaknesses, and areas of opportunity, and build stronger relationships with their communities throughout the process.
Uvalde Memorial Hospital’s (UMH) mission is to promote the healing of those we serve by providing high-quality, compassionate healthcare. This Implementation Strategy reflects UMH’s values of Compassion, Accessibility, Integrity, Stewardship, and Excellence. It builds directly on findings from the 2024 CHNA and establishes a structured, measurable approach for addressing top community health priorities across the five-county service area (Uvalde, Edwards, Kinney, Real, and Zavala counties).
